Home Infusion Pharmacist

Overview

If you're a pharmacist who'd rather be doing real clinical work than counting to thirty, home infusion is worth a look.

Our client runs a home infusion branch in Dublin, OH and needs a licensed pharmacist to join the team. You'd look after the full course of therapy for patients being treated at home – assessing whether a prescribed therapy is right for them, dosing it, compounding it, getting it to them, and keeping an eye on how they're getting on. You'd report to the Pharmacy Manager or Director of Pharmacy.

Specific Skills
 
  • Active pharmacist license in Ohio
  • Either 2+ years in an acute care setting or 1+ year in home infusion
  • Solid IV admixture and IV therapy background
  • Comfortable working alongside physicians and nurses
  • Organized, a good communicator, and happy to be part of the on-call rotation
Responsible For
  • Assessing patients for home care and checking the therapy prescribed is appropriate
  • Reviewing labs and doing pharmacokinetic dosing
  • Processing orders and checking concentration, compatibility and storage
  • Dispensing, and programming and troubleshooting infusion pumps
  • Counseling patients and caregivers on their therapy and how to infuse at home
  • Keeping the clean room and all compounding to USP <797> and state/federal standards
  • Documenting everything properly, including QA/QC and compounding records
  • Handling claims and prior auths when needed
  • Supervising pharmacy support staff and helping train interns and students
  • Taking your turn in the on-call rotation and joining patient care conferences (Nutrition Support Rounds included)
  • Covering as PIC or branch manager now and then when someone's away
Additional Requirements
  • Are you currently licensed as a registered pharmacist in state of employment?
  • Do you have a minimum of two years recent experience in an acute-care setting or one year recent experience in home infusion therapy?
