We’re looking for a Clinical Pharmacist to provide safe, effective, and appropriate medication therapy while working closely with physicians, nurses, pharmacy technicians, and other healthcare professionals.
Specific Skills
- PharmD or Bachelor of Science in Pharmacy
- Active pharmacist license
- Hospital pharmacy experience preferred
- Strong clinical pharmacy knowledge and patient-care skills
- Experience with sterile compounding preferred
- Experience with chemotherapy and/or parenteral nutrition is a plus
- Knowledge of renal dosing, pharmacokinetics, anticoagulation, and clinical interventions
- Strong attention to detail and commitment to medication safety
- Excellent communication and teamwork skills
- Ability to work effectively in a fast-paced hospital environment
- Pharmacy technician supervision experience is a plus
Responsible For
- Review, interpret, and verify medication orders
- Prepare and dispense medications accurately and efficiently
- Monitor patients for appropriate drug therapy, dosing, allergies, contraindications, and drug interactions
- Provide clinical recommendations and medication-related consultation to healthcare professionals
- Participate in interdisciplinary rounds
- Identify, document, and report adverse drug reactions
- Provide medication education to patients, families, and healthcare professionals
- Compound and dispense sterile and non-sterile medications, including chemotherapy and parenteral nutrition
- Safely manage controlled substances and maintain required documentation
- Maintain accurate patient medication records
- Participate in quality improvement, medication-use reviews, audits, and pharmacy initiatives
- Supervise and support pharmacy technicians and pharmacy support staff
- Maintain pharmacist licensure, continuing education, competencies, and required certifications
